If you are a parent of a child that is over 18 years of age your child can apply to the NSW Heath Central Register for information on their donor (see I am donor conceived). Before applying for information NSW Health strongly recommends that you discuss this matter with a counsellor.
Mandatory
Where mandatory information is held on donors whose sperm, eggs or embryos were used to conceive after 1 January2010, identifying information can only be provided in the event of a medical emergency or life threatening situation, where the donor offspring is under 18 years of age. Should this occur parents of those who are donor conceived after 1 January 2010, can apply to the NSW Ministry of Health Director - General for release of identifying information about their child’s donor.
What do I have to do to apply for information held on the Register?
Parents of a child conceived as a result of donated sperm, eggs or embryos after 1 January 2010, can apply to the Central Register for release of mandatory non identifying information as prescribed by regulation held by the NSW Ministry of Health on the donor and any offspring.
To register or apply for information you will need to apply in writing by using one of the application forms on this site. You can also contact the NSW Ministry of Health in person or by phone to request that forms be sent to you.
How will the Ministry of Health let me know?
When an authenticated match has been made the Ministry of Health will notify you via Registered Post that a match has been made. To ensure you are able to receive the result in a supported way, you will also receive information about how to access counselling and the relevant ART Provider.
