If you have no concerns with the outcome of your assessment, it is important you contact your employer AS SOON AS POSSIBLE and advise your employer this is the case.
If you decide you wish to lodge an appeal related to your medical assessment you need to complete the Appeal form, which is on the reverse side of this document.
YOUR APPEAL MUST BE LODGED WITH THE APPEALS COMMITTEE WITHIN 21 CALENDAR DAYS FROM THE DATE OF THE MEDICAL ASSESSMENT NOTICE.
